Dear Customer

It is our utmost priority to ensure the safety of our employees and all shipping agents as well as protect the environment whenever we receive a device from the field back to our facility. Therefore, you are responsible for decontaminating and cleaning completely the instrument before it is returned to our facility and service team.

By ticking the below box, you confirm the following for each device you are returning:

1. The device that I will return to Vögtlin Instruments GmbH (Switzerland) or KEM Küppers (Germany) has been decontaminated and does not present any danger anymore for Vögtlin’s employees, KEM’s employees, the shipping agents or the environment. Especially, the device is free from radioactivity and biohazards and that any residue inside the instrument is not hazardous. The package in which the instrument is sent is free of contamination as well.
2. I understand that failure to decontaminate appropriately the instrument or use a contaminated package may lead to legal claims against me and/or my company as this may harm Vögtlin’s employees, KEM’s employees, the shipping agents or the environment.
3. If the device is a d·flux, I have to fill out the decontamination statement and join it to the delivery papers.
4. I understand that any package that I return without following the required process will not be opened: it will increase the time it takes to process it and extra costs may occur due to the required extra effort to handle and clarify its content.
